* In docs/book/ch13* PASM_QuickRef, I am removing the entries for about 30 old opcodes (i.e. userstack poptop and friends), and adding entries for the ~100 new opcodes. By the end, I should also have code to help keep it in sync with src/ops/*.ops.

- Ticket maintenance and patch application.
- Added a -X command-line option for dynamic extension search paths (modified patch, was -S). Also added long forms --include, --library, and --dynext.
- Made a branch for calling conventions with the first round of changes. Pure PIR subroutine and method calls are working, as are calls to PIR subroutines and methods from C.
- Worked on migrating PCCMETHODs over to the new calling convention internals.
- Edited the first couple of chapters in the book (not checked into trunk until we decide how to handle the difference between the printed book and trunk).
Pynie:
- Worked on implementing print as a function (3.x conversion).
- Worked on implementing keyword arguments (necessary for print as a function).
