Introduce --headers option for --csv-per-line

And make the default to --headers=auto, which produces each line of CSV as a hash, keyed on the header. All other :headers options of Text::CSV are also supported
App-Rak/main: 2e3d2b5927 | (Elizabeth Mattijsen)++ | lib/App/Rak.rakumod
Allow for --headers='<a b c d>'

and --headers=':a<foo>, :b<bar' for mapping
App-Rak/main: cf40ec9841 | (Elizabeth Mattijsen)++ | 4 files
Document the new --headers option
